Kathleen O. broke her maiden on debut in impressive fashion. She rallied from ten lengths off the pace to win a seven-furlong maiden special weight race at Aqueduct on Nov. 12, 2021. Both the runner-up and the third-place finisher in that race came back to win their next start.

In her next start, Kathleen O. won the Cash Run Stakes at Gulfstream Park on New Year's Day 2022. She once again came from behind and pulled away to an 8 1/2-length victory. 

Kathleen O. made her graded stakes debut in the Grade 2 Davona Dale Stakes Presented by FanDuel at Gulfstream Park on Mar. 5, 2022. She drafted behind horses on the turn, then angled into the five-path for clear sailing and won going away to improve to 3-for-3 lifetime. That win earned her 50 qualifying points towards the May 6 Longines Kentucky Oaks, ensuring her of a spot in the starting gate on the first Friday in May.

In the Grade 2 Gulfstream Park Oaks on April 2, Kathleen O extended her career unbeaten streak to four with a last-to-first, 2 ¾-length score over Goddess of Fire. score over Goddess of Fire. The dark bay or brown filly cemented her status as a leading contender for the Longines Kentucky Oaks on May 6 with the victory as she earned another 100 qualifying points.

Kathleen O. dropped back near the back of the field in the early stages of the Longines Kentucky Oaks May 6 at Churchill Downs. She rallied boldly on the turn and into the stretch but flattened out a bit late and suffered her first career defeat in a fifth-place finish in the $1.25-million race.
